The Georgia Department of Behavioral Health and Developmental Disabilities (DBHDD) notified individuals of a data breach when one of their departments laptops was stolen out of a car of an employee who was attending a conference.The laptop contained personal information of over 3,000 patients including names, addresses, phone numbers, dates of birth, names of guardians, marital status, Social Security numbers, Medicaid numbers, diagnosis, behavioral data and other personal information.The laptops were not encrypted.For those who might have been affected, they can call DBHDD at 844-888-5998 until January 9, 2015More Information: http://www.phiprivacy.net/2014/10/page/12/
